What to bring to the Alaska DMV permit test

Bring the right documents and you can take the knowledge and vision tests on the same visit. The official requirements change from time to time, so check the DMV's own page before you go.

For the learner's permit and knowledge test

  • Completed application (Form D1)
  • Parental consent (Form 433 for auto) and proof of relationship to the minor (birth certificate, adoption or guardianship documents)
  • Primary document proving legal name, date of birth and lawful presence (e.g., certified U.S. birth certificate, U.S. passport)
  • Proof of principal residence address
  • Social Security Number
  • $15 fee; pass vision and knowledge tests

Fees

  • Instruction permit (commercial or non-commercial; no REAL ID version): $15
  • Instruction permit renewal: $5
  • Road test (non-commercial, at DMV): $15

The vision test

At least 20/40 in each eye or with both eyes together; corrective lenses must be worn if needed to qualify. A certified statement from a physician or optometrist may be submitted instead of the DMV vision test. Required for original, renewal and duplicate licenses.

Bring your glasses or contact lenses if you wear them to see clearly.

For the road test

  • Instruction permit, or General Knowledge Test scores within one year of passing (18+); under 18: a valid Alaska permit held at least 6 months with no violations in the last 6 months
  • Corrective lenses if you have restriction 1; outside rearview mirrors if you have restriction 2
  • A vehicle in working order (lights with real lenses, windshield without cracks in the field of vision, wipers, defroster, muffler, good tires, brakes including parking brake, mirrors, horn, passenger seat, seat belts for driver and examiner, rear plate with current tabs, speedometer, doors that open inside and out, about 1/4 tank of fuel; no mirrored or aftermarket window tint)
  • Current insurance (printed or on a device) showing the vehicle description and VIN
  • Printed current vehicle registration certificate (or Application for Title and Registration if on a temporary permit); rental car: you must be listed as an authorized driver
  • Under 18: parental consent (parent/guardian in person with a document tying minor to parent, or notarized Form 433)

What happens on the road test

After you pass: your permit

  • Hold it for at least 6 months (applicants 16 or 17) before a provisional license before the road test.
  • Log 40 hours of supervised driving, 10 of them at night.
  • Accompanied by a licensed driver at least 21 with at least one year of driving experience in the same type/class of vehicle, seated beside you (passenger vehicles)
  • Permit valid 2 years; may be renewed once
  • Parental consent (Form D1 back or notarized Form 433) required under 18; signer may become liable for damages and may withdraw consent
  • 40 hours of driving experience certified by parent/guardian/employer, including at least 10 hours in progressively challenging circumstances (inclement weather, nighttime)
  • No traffic law convictions in the 6 months before applying for a provisional license
  • IE permit: valid only with a licensed driver training school instructor

Before you go

Take a practice test or two the day before, and the exam simulator to check you can pass in the real format. Bring originals, not photocopies, unless the DMV says copies are accepted.
